Agreement

This contract is an agreement between ForeverNow LLC (“Practitioner”) and you (“Client”) for the purpose of implementing a smoking cessation program.

Services Provided

The Practitioner will provide coaching in techniques that assist with the cessation of smoking. The program includes the following:

  • 24/7 Online Training with Lifetime Access
  • Email Support for 1 year
  • Ongoing Group Support
  • Training in visualization, NLP, self-hypnosis, habits, and other topics
  • Elite Access Only: Eight (8) 30-minute Virtual Coaching Sessions

Additional coaching sessions can be purchased as needed.

Client Responsibilities

The Client will complete the training modules and utilize the support resources. The Client agrees to use the 5-minute Craving Hack every time he/she has a craving before choosing to smoke. The Client understands that success is determined by their own efforts and that the Practitioner cannot guarantee success. The Client commits to being 100% committed to quitting smoking.

Mandatory Requirements for Complete Success

The Client agrees to remove all tobacco products from their home and car prior to the first session. The Client agrees to completely clean their home and car of all cigarette smells. The Client agrees to start their smoking cessation program as a non-smoker and with a clean slate. The Client agrees to start a new life, free of all things to do with smoking.

Fees and Refunds

The fee for the program is non-refundable. This fee covers all the training, coaching, and assistance as described.

Limitation of Liability

The Practitioner shall not be liable for any direct, indirect, incidental, special, consequential, or exemplary damages, including but not limited to, damages for loss of profits, goodwill, use, data, or other intangible losses resulting from the use of or inability to use the service.

Intellectual Property

All materials provided are the intellectual property of ForeverNow LLC and are for the Client’s personal use only. Unauthorized distribution is prohibited.

Governing Law

This Agreement shall be governed by the laws of the State of Georgia in which ForeverNow LLC operates.

Entire Agreement

This Agreement constitutes the entire agreement between the parties and supersedes all prior and contemporaneous agreements, proposals, or representations, written or oral, concerning its subject matter.

Acceptance of Terms

By joining this program online or in person, you, the Client, agree to the conditions set forth herein and commit to doing all that is necessary to stop smoking.
